Smoking and Back Pain Related Problems

Abstract
Background: There are various known occupational and environmental factors that might be related to the development of low back pain. The intention of this study was to review the relationship between smoking and low back pain related problems with regards to the epidemiological and biological points of view to be useful guidelines in clinical settings.

Materials and Methods: All publications from medline were retrieved for the smoking and back pain and these were sorted out for the well designed studies. There were total of 41 review journals that were met for this purpose and 14 of review journals were selected for the close evaluation.

Results: Among 41 journals, 32 have demonstrated that there were strong positive relationships between smoking and low back pain. Majority of remaining 11 also show tendency of positive relationship between smoking and development of low back pain but conclusive remarks were not drawn. Rather, only weak correlations and indirect relationships were consistent in these studies.

Conclusions: These results indicate that smoking is considered to be directly related to the development of low back pain and its causes may be multifactorial. Some of important known causes are disturbances of bone or mineral metabolism and circulation. However, other factors, such as psycho-social factors, may also be might be closely related to the development of low back with smoking. In these setting, there will be even greater burden for both patients and society for the management of these problems. Appropriate recognition of factors related to the development and aggravation or becoming chronic would be mandatory as well as further studies on verification of relationship between factors in development chronically disabling low back pain and optimal management of these patients with regards to best treatments and timing of return to previous of works.
